29 CFR 1917.112(c)(3): The top surface of guardrails installed after October 3, 1983, was not 42 inches, plus or minus 2 inches high:    a) Barge Access Platform: On or about July 15, 2015, the employer did not ensure the wire rope top rail was maintained and used at the required height which exposed employees of a fall of 9-feet to the shallow water below.

29 CFR 1917.112(c)(4): Non-rigid railing, such as chain or wire rope, had a maximum sag of more than 6 inches at the mid-point between posts:  a) Barge Access Platform: On or about July 15, 2015, the employer did not ensure the sag in the wire rope standard railing system was less than 6-inches which exposed employees of a fall of 9-feet to the shallow water below.
