29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Production Area- The employer did not ensure that employees were adequately protected from hazards created by rotating parts and ingoing nip points, such as those created by the take away conveyors on Lollipop Machine Lines 7 and 9.    Primrose Candy Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 111159, citation number 1, item number 1 and was affirmed as a final order on March 13, 2012, with respect to the workplace located at 4111 W. Parker Avenue in Chicago, Illinois 60639.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of the incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on the OSHA public web site at www.osha.gov:      a) The employer did not report an in-patient hospitalization that was the result of a work-related incident to OSHA within twenty-four (24) hours for an injury that occurred on July 15, 2015.
