On or About July 22, 2015 an employee of Carlisle Constructors, Inc was laying and nailing plywood to form the roof structure of the project and was working on the leading edge of the roof system approximately 16 feet above the ground and was exposed to a fall of over 16 feet because he was not protected by any means of a guardrail system, safety nets, or personal fall protection systems.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) On or about July 22, 2015, Employees of Carlisle Constructors Inc. were working on the roof surface of the project. The roof surface was scaled off the blueprints to be 16 feet above the finished floor. The exterior wall top plate was 13 feet above the finished floor. The roof trusses, were 3 feet tall.  Employees were using a 16 foot portable ladder to access the roof surface, the top rails of the ladder only extended to the top plate of the exterior wall. CSHO observed employees climbing from the top rung of the ladder through the truss system to gain success to the roof.

Standing on a 6 foot A frame folding ladder leaning against the exterior parapet wall of the project to reach the top of the parapet wall.  Leaning the ladder against the wall increases the chances of the ladder slipping out from under the load of the employee standing on it.

On or about July 22, 2015 CHSO observed employees working throughout the project completing various tasks such as climbing ladders, framing parapet walls, constructing roof surfaces, layout and erecting exterior walls. Employees were not protected from puncture wound to the feet, or other foot injuries because they were not wearing appropriate work boot protective foot wear.  Employees were wearing rubber sole sneakers on the job site.
