29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:    a.   Employees working at elevations greater than 6 feet from the ground or lower levels while performing residential framing are not being protected from fall hazards by a fall protection system.  The employer fails to assure effective fall protection against falls as required by the standard as most recently demonstrated on June 29, 2017, at a job site at 259 Rock Spring Loop in Saint Augustine Florida, on the 5:12 pitch roof of the house when framing employees installing plywood roof sheathing were exposed to 10 feet to 18 feet fall hazards from the roof eaves and down through the open truss framing, in that they were not protected by a fall protection system.    Carlisle Constructors, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard, 29 CFR 1926.501(b)(2)(i), which was contained in OSHA inspection number 1079171, Serious citation number 01, item number 001, and was affirmed as a final order on August 29, 2015, with respect to a workplace located at 12697 Bartram Parkway Blvd, Jacksonville, Florida.    Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including and explanation of how these steps protect its employees from fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a.   On or about June 29, 2017, at the front of the house, the roof access ladder did not extend 3 feet above the eave at the upper landing surface, exposing employees to a 10 feet fall hazard.    Carlisle Constructors, Inc. was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1079171, Serious citation number 01, item number 002a, and was affirmed as a final order on August 29, 2015, with respect to a workplace located at 12697 Bartram Parkway Blvd, Jacksonville, Florida.

29 CFR 1926.302(a)(1): The noncurrent-carrying metal parts of electrical power operated hand tool(s) were not grounded, or of the approved double insulated type:  a.   On or about June 29, 2017, at the job site, the Makita Circular hand saw, a double insulated tool, had been modified by opening the saw's casing and replacing the manufacturer's power cord with a 3 prong, 50-foot extension cord.

On or about June 29, 2017, employees working at elevations above 6 feet in the trusses and on the roof had not been trained on proper selection, installation, and use of personal fall protection anchor devices.
