29 CFR 1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made.  a. North Shop Building - On or about July 30, 2015, a portable lamp which cord had significant damage to the outer insulation was not removed from service, thereby employees were exposed to the hazards of electrical shocks.

29 CFR 1910.132(e): Defective and damaged equipment. Defective or damaged personal protective equipment shall not be used.  a. North Shop - On or about July 23, 2015, the employer permitted welding operators to use damaged and/or defective personal protective equipment, thereby employees were exposed to the hazards of gas and metal welding operations.  1. Jackson Black Helmet - an employee had placed black tape over loose and broken/fragile plastic head band piece.  2. Jackson Black helmet - an employee was continuing to use a perforated/burned welding hood helmet.

29 CFR 1910.252(c)(1)(ii): Screens. When welding must be performed in a space entirely screened on all sides, the screens shall be so arranged that no serious restriction of ventilation exists. It is desirable to have the screens so mounted that they are about 2 feet (0.61 m) above the floor unless the work is performed at so low a level that the screen must be extended nearer to the floor to protect nearby workers from the glare of welding.  a. North Shop/Small Piece Welding Area - On or about July 23, 2015, the employer had a welding area that was not provided with at least four (4) screen walls. Employees walking along the area were exposed to nonionizing radiation  hazards.  The conditions of the welding area included:  1. The one (1) portable screen available in the area had its curtain off the metal frame (top and bottom). In addition, the metal bottom portion of the frame was twisted and out of its intended shape.    2. Three (3) other portable screens were missing from the area.
