On or about July 22, 2015, an employee was using a Donkey Truck Carried Forklift to move concrete cast moldings from the fabrication shop to the outside storage areas and was not provided with formal instruction and an evaluation of the operators performance prior to using the Donkey Truck Carried Forklift.
