At RJ Staab Stone Company of Florida, LLC's warehouse located at 390 South Seaboard Drive, Welborne Florida, 32097: On or about October 10, 2023, and at times prior, the employer exposed employees to struck-by and crushed-by hazards, in that the employer did not certify that each operator has been trained and evaluated as required by this paragraph (l). RJ Staab Stone Company of Florida, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1440921, citation number 3, item number 1 and was affirmed as a final order on April 23, 2020, with respect to a workplace located at 824 North Main street, Willison Florida, 32696.
