29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person:   On or about July 22, 2015, and at times prior thereto, Employees worked from a job made scaffold that was not constructed in accordance with the design by a qualified person. Therefore, the scaffold design lacked the required capacity ratings, safety factors and stability assurance.  Employees were exposed to the hazard of falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about July 22, 2015, and at times prior thereto, employees engaged in framing activities at residential construction site, working at heights greater than 6 feet above lower level, were not protected by guardrail systems, safety net systems, or personal fall arrest systems.  Employees were exposed to the hazard of falls.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:   On or about July 22, 2015, and at times prior thereto, employees were not provided a ladder for access/egress from ground level to working levels.  Employees climbed the framing to access and egress from the residential framing construction.  Employees were exposed to the hazard of falls.
