29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. A. APBN, Inc. worksite located at I 475/23 & Bancroft St., Sylvania, Ohio: On or about July 20, 2015, the employer did not ensure employees were protected from a fall hazards while working from a suspended platform performing bridge painting activities. A employee fell approximately 15.00 feet and sustained serious injuries due to the lack of fall protection. APBN, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451 which was contained in OSHA inspection number 313785396, citation number 2, item number 1 and was affirmed as a final order on July 2, 2012, with respect to a workplace located at Toledo, Ohio.
