29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) On or about 07/19/2015 in building 2540, an employee was exposed to an amputation hazard, in that, lock out or tag out devices were not used while working at press #3 conveyor. An employee's fingers were caught in between the belt and gearbox sprocket when trying to start-up a pulley belt by pulling on it.
