29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    (a)  SANITATION DEPARTMENT - Employees cleaned and sanitized kitchen equipment that is used for food production, where their assigned sanitation tasks required the equipment to be locked and tagged out of service during the process, and the employee(s) had not been provided training on how or when to utilize lockout tagout procedures, thereby exposing the employees to severe hazards that included amputation injuries.  An employee reached into a running piece of equipment with a 12 inch diameter auger in order to clean out pieces of potatoes, and the employee had two fingers on his right hand amputated, on or about July 14, 2015.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
