29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to, the requirements noted in the 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): A.) Walker Lumber Company, Inc. - LOTO Procedures: On or about 7/23/15, employees conducted servicing and maintenance work on machinery that included but was not limited to debarkers, saws, kilns, and conveyors. The specific lockout/tagout procedures were missing items that included but were not limited to (1) Magnitudes of the energy sources (2) Specific techniques/instructions for bleeding pneumatics and other air sources (3) Specific instructions on the detailing the manner in which energy isolation was to be verified on the machine. (4) In addition, the specific procedure for the debarker was lacking specific instructions for bleeding the pneumatic energy source.

Walker Lumber Company, Inc. -- Green Chain Conveyor: On or about 7/23/15, the shaft at the end of green chain conveyor that powers the conveying sprockets/chains was not guarded. The shaft was located approximately 3 to 4 feet above the ground.

Walker Lumber Company, Inc. -- Green Chain Conveyor: On or about 7/23/15, the red colored metal guards at the end of green chain conveyor were damaged bent and twisted. The guards cover the ends of the chain/sprocket assembly. The damaged metal guards created a potential laceration hazard.
