A conduit leading into a junction box with five missing knockouts was  broken off and separated from the junction box.           c) Jobsite, 780 Reservoir Road, Hollidaysburg, PA 16648:  Observed on or about October 16, 2018, employees were exposed to electrical hazards created by an electrical panel that was missing screws to secure the front cover to the frame of the panel.  An orange flexible cord ran through a gap between the front cover and the frame of the electrical panel.
