29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.  (a) Located on the Community Center roof at the Steele Center Compound in Crater Lake National Park, an employee patching the roof secured his rope lanyard to a vertical pipe by wrapping the rope around the pipe and hooking his snaphook back to the rope lanyard. The snaphook was not designed for this application, potentially exposing employees to death from failure of the system if they were to fall to the lower level of about 24 feet.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.   (a) Located on the Community Center roof at the Steele Center Compound in Crater Lake National Park, employees patching the roof used a piece of vertical pipe protruding from the roof as an anchor point. The configuration of the pipe did not allow for a means to prevent disengagement of the personal fall arrest system in the event that fall were to occur, potentially exposing employees to death from a fall to the lower level of about 24 feet.
