29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  Crater Lake Housing Unit 226:  On or about July 21, 2015 and times prior thereto, an employee was using an aerial lift working at approximately 10-12 feet in height. Employee had a fall protection harness with a lanyard of six feet with a deceleration of 3 feet 10 inch..  This exposed the employee to struck-by hazard since he would hit the ground from that height.
