29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by ingoing nip points and rotating parts: a) Stitch and Trim Area, 775 Indiana Springs Road, Indiana, Pennsylvania 15701: Observed on or about July 23, 2015, a guard on the three knife trimmer section of a Mueller Martini stitching and trimming machine was not attached. Rotating parts near the output side of the machine were exposed. Employees using the machine were exposed to caught-by hazards from ingoing nip points.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: a) Harris Press area, 775 Indiana Springs Road, Indiana, Pennsylvania 15701: Observed on or about July 23, 2015, the flexible cable leading into a pendant style electrical outlet box (110 volt) was not connected to the outlet box such that tension would not be transmitted to the joints or terminal screws. Employees using the outlet box were exposed to the hazard of electrical shock.
