29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):  Employee(s) performing tasks with the vertical velocity fan damper in building 2 were exposed to caught in between hazards due to the 5 inch diameter pulley of the equipment not being fully enclosed.  The current guard had an inside opening of approximately 8 inches by 4-1/2 inches. The front side of the guard had a 5 inch diameter opening that was connected by a 3 inches by 1 inch section that was not guarded.  The vertical velocity fan motor was operating at 1,750 revolutions per minute.
