29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:    a) Packaging Department - The employer did not ensure that the workplace was kept in an orderly condition as there was an electrical extension cord powering a fan placed on the floor across the aisle that created a tripping hazard.        b) CNC Department - The employer did not ensure that the workplace was kept in an orderly condition as there was a pneumatic air line hose used for compressed air placed on the floor across the aisle that created a tripping hazard.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    a) The employer did not establish an energy control procedure (lockout/tagout program) and did not provide training on the energy control procedure to ensure all required elements of the procedure were understood by employees and the knowledge and skills required for the safe application, usage and removal of energy controls were acquired by employees performing servicing and maintenance on a machine or equipment, such as, CNC machines, mills, cyclical grinders, and lathes.

The Allis-Chalmers forklift, model #C50LPS, serial #AMO522423, did not have non-slip covers on the metal foot pedals to prevent the operator's foot of slipping off.
