29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about July 30, 2015, the employer did not ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, tooling of the line 1 and 2 Tube mills. Employees were exposed to amputation injuries during set up and and operation.
