29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition.  a) 1200 St Johns Rd, Camp Hill, PA - On or about July 31, 2015, the floors in front or and/or leading to exit doors to the outside at the northwest loading dock man door were not kept in an orderly condition as evidenced by several items discarded on the floor such as broken wooden pallets, cardboard and trash, which were exposing employees to a tripping hazard.  b) 1200 St. Johns Rd, Camp Hill, PA -  On or about July 31, 2015, the floors in front of and/or leading to exit doors to the outside at the southeast loading dock man door were not kept in an orderly condition as evidenced by several items discarded on the floor such as broken wooden pallets, cardboard and trash, which were exposing employees to a tripping hazard.   c) 1200 St. Johns Rd., Camp Hill, PA - On or about July 31, 2015, the floors in front or and/or leading to the outside of the southeast main building lower level break room man door were not kept in an orderly condition as evidenced by several items discarded on the floor such as broken wooden pallets, cardboard and trash, which were exposing employees to a tripping hazard.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

Materials or equipment were placed, either permanently or temporarily, within the exit route.  a) 1200 St Johns Rd, Camp Hill, PA - : On or about July 31, 2015,  the loading dock northwest door exit routes was found obstructed due to extraneous material including discarded wood pallets and trash.    b) 1200 St. Johns Rd., Camp Hill, PA - On or about July 31, 2015, the loading dock southeast door exit route was found obstructed due to extraneous materials including discarded wood pallets and trash.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.22(a)(3): Floors, working places, and passageways were not kept free from protruding nails, splnters, holes, or loose boards.  a) 1200 St Johns Rd, Camp Hill, PA - Loading Dock: On or about July 31, 2015, the outside plywood floor landing for the lower level break room exit door was not maintained in good repair.  Due to deterioration and rot, holes have developed in this passageway utilized by employees.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.157(c)(1): The employer did not provide portable fire extinguishers and did not mount, locate, and identify them so that they were readily accessible to employees without subjecting the employees to possible injury:  a) 1200 St Johns Rd, Camp Hill, PA - Loading dock:  At the Northwest area of loading dock a portable fire extinguisher was not readily available. Access to the ABC fire extinguisher was blocked by trash and debris on or about July 31, 2015.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.
