B, Shop - A DeWalt double wheeled bench grinder (MD. DW758) had measured gaps of 3/4" on the right side and a 1/2" gap on the left side between the work rests and the abrasive wheels.

B, Shop - A DeWalt double wheeled bench grinder (MD. DW758) had measured gaps of 1" on the right side and a 1/2"  on the left side between the tongue guards and the abrasive wheels.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.   Bldg. B, Shop - Welding gases Oxygen and Acetylene were stored together when not in use.  The distance between the stored gases was 7'6".

29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account  of fixed machinery, equipment or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or maerials from falling through:  Bldg. A, OMAG color hopper, second level  - At the end of the color hopper line, a section of grating in excess of one inch wide was missing.

29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.     Bldg. A, South East exit door -- It was observed that that the exit discharge was blocked by sand bags.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:          Workplace - The employer failed to train exposed employees on the new label elements (e.g., pictograms and signal words) and the new SDS formation by December 2, 2013, as required by the revised Hazard Communication Standard.  Employees handle and use hazardous chemicals such as, but not limited to,  Portland cement, welding gases (Oxygen, Acetylene, ), fuels, and mineral pigments.
