29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about July 30, 2015, employees who were exposed to fall hazards were not trained in fall protection procedures to recognize and minimize those hazards. The Nu-Life Enterprises was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a(1), which was contained in OSHA inspection number984555, citation number 1, item number 2 and was affirmed as a final order on May 27, 2015, with respect to a workplace located at 703 South Adams Street, Portage, WI 53901. ABATEMENT DOCUMENTATION IS REQUIRED FOR THIS ITEM IN ACCORDANCE WITH REQUIREMENTS OF 29 CFR 1903.19(D).

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about July 30, 2015, employees were painting a low slope roof with no fall protection. ABATEMENT DOCUMENTATION IS REQUIRED FOR THIS ITEM IN ACCORDANCE WITH REQUIREMENTS OF 29 CFR 1903.19(D).
