29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use [ANSIZ358.1-2004]:    a. On or about 07/23/15, at the warehouse area, at Cost-U-Less, St. Thomas, VI. An emergency eye wash station was not provided in the immediate area when employees used and/or handled corrosive products such as Deka batteries (sulfuric acid) when checking water levels and/or replacing them during Crown lift truck maintenance operations. A Sperian emergency eye wash station located in the warehouse area was obstructed with boxes and a grocery cart and the cartridges had expired on 5/2014.  NOTE:  Certification of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c).

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  On or about 07/23/15, at the warehouse area, at Cost-U-Less, St. Thomas, VI. The employer did not implement their written hazard communication program to ensure identity/warning labeling and safety data sheets availability as follows:  a. Employees used and/or handled compressed gases such as propane tanks when replacing empty tanks with new tanks during Komatsu forklift maintenance operations.     b. Employees used and/or handled corrosive products such as Deka batteries (sulfuric acid) when checking water levels and/or replacing them during Crown lift truck maintenance operations.   NOTE:  Certification of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c).

On or about 07/23/15, at the receiving area, at Cost-U-Less, St. Thomas, VI. The employer did not ensure to place identity and hazard warning labels on a 1,000 gallon tank container for diesel storage used and/or handled by employees as fuel source for the power generator.
