29 CFR 1910.24(h): Standard railings were not provided on the open sides of all exposed stairways and stair platforms.    a). Men's Toilet Facilities - On or about and times prior to July 29, 2015, the stairway leading to toilet facilities was not equipped with standard railing on the left and right open sides ascending, thus exposing employees to a trip and a fall hazard of approximately 2.5 feet onto earthen ground.    b). Production Manager's Office - On or about and times prior to July 29, 2015, the stairway and its platform leading to the production manager's office was not equipped with intermediate railing on the right side ascending, thus exposing employees to a trip and a fall hazard up to approximately 10.75 feet onto concrete flooring.    c). Exterior and Tops Department - On or about and times prior to July 29, 2015, the stairways on the mobile scaffolding units were not equipped with intermediate railing on the left and right sides ascending, thus exposing employees to a trip and a fall hazard up to approximately 8.75 feet onto concrete flooring.
