29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine areas from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Cooper Tire & Rubber Co. located at 701 Lima Ave., Findlay, OH: On or about June 26, 2015, the employer did not assure that proper guarding was in place while an employee was attempting to align a prox switch on top of the press when it unexpectedly activated and crushed his hand on the black collar.

Cooper Tire Company located at 701 Lima Ave. Findlay, OH; On or about June 26, 2015, the employer did not assure Tire Press P-5 was deenergized before performing servicing activities. An employee was attempting to align a prox switch on top of the press when it unexpectedly activated and crushing his right hand on the black collar.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: a.) Cooper Tire Company located at 701 Lima Ave. Findlay, OH; On or about June 26, 2015, the employer did not assure employees were trained to understand and perform energy control procedures. Employees are required to adjust prox switches on the SMO for Tire Presses, including but not limited to Tire Press P-5 and are exposed to crushing hazards.
