Noted on or about 07/31/15 b - Underneath Bridge 1: Company employees were exposed to struck by and caught in hazards when they had not received any training from the company regarding the recognition and elimination of hazards when using come alongs/wire rope hoist(s) at the worksite. Noted on or about 07/31/15. c - Underneath Bridge 1: Company employees were exposed to struck by and caught in hazards when they had not received any training from the company regarding the EFCO Plate Girder Bridge manufacturer's assembly requirements. The soil beneath the excavator's tracks showed clear signs of ravelling.
