29 CFR 1926.501(a)(2): The employer did not determine if the walking/working surfaces on which its employees were to work had the strength and structural integrity to support employees safely:  a) Jobsite - The employer designed a system made of plywood type material and lumber, and instructed an employee to stand on the platform to receive materials from the floor below.  The system collapsed under the weight of the employee and the materials being lifted causing the employee to fall approximately eleven (11) feet.  Observed on or about 11 JUNE 2015.
