Pleasant, PA: On or about 08/03/15 employees who go on top of the trailers to install trim molding are not provided with fall protection. The fall hazard from the top of the trailer to the floor is approximately 10 1/2 feet.

29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses: (a) Facility, 480 Bessemer Road, Mt. Pleasant, PA: On or about 08/03/15 employees who were prescription glasses were not provided with eye protection that incorporated safety lenses or eye protection that could be worn over their prescription glasses. Employees work with grinders, saws and other hand tools and have exposure to flying chips or debris.
