29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  On or about August 3, 2015 and times prior thereto, at the worksite located at or near 1010 Jessie Road in Little Rock, Arkansas, employees were allowed the voluntary use of dust masks. The employer did not provide the information from Appendix D to the employees. This condition exposed employees to inhalation hazards.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:(a)  On or about August 3, 2015 and times prior thereto, at the worksite located at or near 1010 Jessie Road in Little Rock, Arkansas, the employer did not have a written hazard communication program for the materials used in the facility, to include but not limited to silica from the marble and other stones. This condition exposed employees to chemical hazards.
