29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: On or about August 3, 2015 and times prior thereto, at the worksite located at or near 1010 Jessie Road in Little Rock, Arkansas, employees were allowed the voluntary use of dust masks. The employer did not provide the information from Appendix D to the employees. This condition exposed employees to inhalation hazards.

This condition exposed employees to chemical hazards.
