29 CFR 1910.22(c): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc.: On or about January 6, 2016, and times prior to, the water sump pit located at the back of the shop was not completely covered. This condition exposed employees to slips, trips and falls into the pit.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: On or about January 6, 2016, and times prior to, employees voluntarily wearing 3M N95 disposable respirators were not provided with a copy of Appendix D. This condition exposed employees to health hazards. Advanced Bath and Kitchen, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(k)(6), which was contained in OSHA inspection number 1082255, citation number 1, item number 2 and was affirmed as a final order on September 24, 2015, with respect to a workplace located at 1010 Jessie Road in Little Rock, Arkansas.
