29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   (a)  Jobsite:  On or about August 4, 2015, an employee was working while standing on the mid-rail of a scissor lift.  The fall hazard to the employee was about 14 feet to a concrete floor.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761:  (a) Jobsite:  On or about August 4, 2015, the employer did not train each employee that was exposed to fall hazards while installing insulation and a standing seam roof.   The fall hazard to the employees was up to over 16 feet.

29 CFR 1926.760(b)(3): Each connector working at heights over 15 and up to 30 feet above a lower level, was not provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or was not provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section:    (a)  Jobsite:  On or about August 4, 2015, three employees were working at installing the standing seam metal roof (and insulation) and at times were exposed to falls over 15 feet and the company did not have any fall protection on the roof (nor on the site), and there was no controlled access zone established.  The fall hazard to the employees was up to over 16 feet to a concrete floor.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:   (a)  Jobsite:  On or about August 4, 2015, an employee was using a 100 foot electrical extension cord that had its grounding pin pulled.   This employee was using a DeWalt electrically powered tool.
