29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:  Construction site: On or about February 25, 2016 and at times thereto, two employees were observed working from the top rails of scissor lifts which was not a working surface of at least 18 inches wide. This condition exposed the employees to a fall greater than 10 feet.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  Construction site: On or about February 25, 2016 and at times thereto; the employer did not provide a training program to enable employees to recognize the hazards of falling while working from the top rails of a scissor lift. This condition exposed the employees to a fall greater than 10 feet.

29 CFR 1926.760(b)(3): Each connector working at heights over 15 and up to 30 feet above a lower level, was not provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or was not provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section:  Construction Site: On or about February 25, 2016 and at times prior thereto, one employee was observed standing on a beam with out any fall protection. This condition exposed the employees to a fall hazard greater than 15-feet to the ground below.  Performance Steel, LLC was previously cited for a violation of this occupational safety and health standard CFR 1926.760(b)(3), which was contained in OSHA inspection number 1082375, citation number 01, item number 003 and was affirmed as a final order on September 25, 2015, with respect to a workplace located at 911 Main Street, Hickory, PA 15340.
