Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. a) On or about August 4, 2015, during grinding activities on the Hammons KEF, Model #BSH75, located in the warehouse area, the employer did not provide, allow, and maintained personal protective equipment for the hands when employees were exposed to cut and amputation hazards.

The electrical flexible cord was damaged exposing employees to electrical shock hazards.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: a) On or about August 4, 2015, located in the center of the warehouse, the employer did not ensure that the portable fire extinguisher was readily accessible to employees. The portable fire extinguisher was blocked by carts, boxes, pallets, a punch press and other materials. b) On or about August 4, 2015, located behind the Accurpress Serial #6754 in the warehouse, the employer did not ensure that the portable fire extinguisher was readily accessible to employees. The portable fire extinguisher was blocked by steel beams behind the Accurpress, cords, and a cart. The Kiefer Industries Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 574059, citation number 1, item number 2a and was affirmed as a final order on January 8, 2013, with respect to a workplace located at 400 Industrial Dr., Random Lake, WI 53075.
