29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing.    Lakeview Farms, LLC - Delphos, Ohio: On or about August 5, 2015, employees that fill hoppers at the 4-Out ATS machine were exposed to laceration and abrasion injuries in that the walkway adjacent to the machines conveyor was not guarded by a standard guardrail.

Employees that maintained the machine were not trained to isolate energy sources.

Employees cleaning the machine were exposed to pinch point and amputation injury in that the pneumatic and mechanical energy sources were not isolated.
