Lakeview Farms, Inc. - Delphos, Ohio: On or about February 7, 2025, the employer did not ensure that employees who worked from a JLG scissor lift (model 3246ES) were trained in the recognition of hazards associated with this type of mobile scaffold.  Untrained employees who operated the scaffold in the Chopper Corridor to access a second level wall panel opening to the parts room, were exposed to fall and trip/fall hazards in that the mid-rail on the scaffold's access gate was not in place and transitioning to/from the scaffold was not made through the scaffold's access gate.
