29 CFR 1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Lakeview Farms, Inc. - Delphos, Ohio: On or about February 7, 2025, the employer did not ensure that employees who worked from a JLG scissor lift (model 3246ES) were trained in the recognition of hazards associated with this type of mobile scaffold. Untrained employees who operated the scaffold in the Chopper Corridor to access a second level wall panel opening to the parts room, were exposed to fall and trip/fall hazards in that the mid-rail on the scaffold's access gate was not in place and transitioning to/from the scaffold was not made through the scaffold's access gate.
