29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person: On or about August 03, 2015, employees conducting framing and decking activities worked from a job made scaffold that was not designed or constructed by a qualified person, therefore, the scaffold design lacked the required capacity ratings, safety factors and stability assurance. Employees were exposed to the hazard of falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about August 3, 2015, an employee engaged in residential framing and decking, working at heights greater than 6 feet above lower level, was not provided fall protection. Employee was exposed to the hazard of falls. Jose Rodriguez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 965751, citation number 1, item number 1a and was affirmed as a final order on September 17, 2014, with respect to a workplace located at 7634 N. 181st East Ave., Owasso, OK 74055.
