29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:  a)  Auditorium Mezzanine;  The railing measured 36 inches high with a 15 foot 4 inch fall to the lower level.   b)  Four breezeways had railings which measured 37 inches high exposing employees to more than a 15 foot fall.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a)  Aqua Lab West;  A relocatable power tap powered a water/air pump adjacent to pools of water.   b)  Auditorium;  Metal handy boxes made into heavy duty extension cords were not mounted and the box was moveable.   c)  Library;  An electric kettle was plugged into an  extension cord.   d)  Maintenance Managers Office;  A relocatable power tap was plugged into another relocatable power tap.   e)  Locker Area, Maintenance Shop;  A Keurig coffee/tea maker, DeLonghi toaster over and Kenmore microwave oven were plugged into a relocatable power tap which was plugged into an extension cord (daisy chain).   f)  Room 332 East;  A Sharp microwave and brown mini-refrigerator were plugged into a relocatable power tap.   g)  Conference Room 438 East;  A relocatable power tap was plugged into a handy box extension cord (daisy chain), which powered a polycom phone, TV and other audio visual equipment.    h)  Conference Room 438 East;  A handy box extension cord was plugged into an extension cord (daisy chain).   i)  Room 402 East;  A portable heater was plugged into a relocatable power tap. Room 102 South;  An electric kettle was plugged into a relocatable power tap.   j)  Room 216 South;  A Therapure dehumidifier was plugged into a relocatable power tap.   The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  This exposes employees to possible electrical fire hazards.      The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.
