Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) August 4, 2015, on the roof of the Shell Barrigada Foodie's building, an employee was not protected from a 12-foot fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On August 4, 2015 at the east side of the Shell Foodie's building, an extension ladder setup to access the roof did not extend at least three feet above the roof, exposing employees to the hazard of falling from a height of about 12 feet to the ground.
