29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a) On the roof of Building II, at least six (6) employees, who were performing detail roofing work (sealing, flashing, and installing vent caps, etc.), were not protected from falling through the ladder access opening, which was approximately 12-feet 5 1/2-inches to the lower level.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage and other deterioration, and defective components were not removed from service:  On the roof of Building II:  a) The synthetic rope lifeline that the employer installed east of the ladder access opening for the employees' use had torn strands and was not removed from service.  b) The 18-inch nylon lanyard that the employer provided for the employees' use was torn and not removed from service.

29 CFR 1926.1053(b)(1): When a portable ladder was used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  Employees were using a wood job made ladder that did not extend 3-feet above the landing were exposed to fall hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  a) On the roof of Building II, at least one (1) employee, who was securing the roofing material on top of the trash chute, was not protected from falling approximately 32-feet, 3 3/8-inches to the lower level.  b) On the roof of Building II, at least six (6) employees, who were performing detail roofing work (sealing, flashing, and installing vent caps, etc.), were not protected from falling approximately 30-feet, 3 3/8-inches to the lower level.  QUICK ROOFING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 1926.501(B)(11) OR ITS EQUIVALENT WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314179961 CITATION NUMBER 1, ITEM NUMBER 1A THAT WAS ISSUED ON DECEMBER 06,  2010 WITH RESPECT TO A WORKPLACE LOCATED ALMA DR AND HEDGCOXE RD 75013.  THE FINAL ORDER DATE WAS DECEMER 06, 2010.
