29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) On the roof of Building II, at least six (6) employees, who were performing detail roofing work (sealing, flashing, and installing vent caps, etc.), were not protected from falling through the ladder access opening, which was approximately 12-feet 5 1/2-inches to the lower level.

QUICK ROOFING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 1926.501(B)(11) OR ITS EQUIVALENT WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314179961 CITATION NUMBER 1, ITEM NUMBER 1A THAT WAS ISSUED ON DECEMBER 06, 2010 WITH RESPECT TO A WORKPLACE LOCATED ALMA DR AND HEDGCOXE RD 75013. THE FINAL ORDER DATE WAS DECEMER 06, 2010.
