29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. On or about August 5, 2015, an employee working from an aerial lift basket had not attached the lanyard to an anchor point located on the working platform. This exposed the employee to a fall greater than six (6) feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about August 5, 2015, the employer had not provided training to exposed employees in recognizing fall hazards and the procedures to follow in order to minimize fall hazards. This exposed the employee to a fall greater than six (6) feet to the ground below. Juan Francisco Chajon was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1), which was contained in OSHA inspection number 315625509, citation number 1, item number 6, and was affirmed as a final order on July 28, 2011, with respect to a workplace located at 6212 Crow Lane, Austin, Texas 78745.
