29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): (a) A Clark powered industrial truck, model ECS 27, serial # 357-0616-8975, being used to lift press dies into position, was not maintained in safe operating condition when the rear right support arm of the FOPS was found to be bent. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

ABATEMENT CERTIFICATION IS NOT REQUIRED FOR THIS ITEM.
