On August 7,2015, employees without fall protection were exposed to fall hazards of approximately 10.8 feet above the waters of the DuPage River while erecting precast concrete bridge deck planks. Copenhaver Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 949716, citation number 1, item number 2 and was affirmed as a final order on February 26, 2014, with respect to a workplace located at 5 Points Road Bridge over Kishawaukee River Kingston, Illinois. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
