29 CFR 1904.29(b)(1) as required by 1960.8(b): The log of occupational injuries and illnesses, (OSHA Form No. 300), was not completed in the detail provided in the form and the instructions contained therein: U.S. Dept. of Homeland Security, Immigration and Customs Enforcement, Homeland Security Inv. - 1 Federal Drive, Suite 1340, Fort Snelling, MN. On or about August 3, 2015, the employer had not correctly and completely entered workplace injuries or illnesses on the OSHA Form 300 for calendar year 2015. (a) On or about April 29, 2015, an employee received an injury in the hip, case number 17814, which resulted in two days away from work. The employer classified the case as an other recordable case rather than a case involving days away from work.
