29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:    During the week of November 6, 2015 and prior the employer failed to ensure employees who were performing maintenance work were protected from various fall heights, including but not limited to, the following cranes:    a. An approximate 40 foot fall on the trolley of crane 304 by utilizing an effective guardrail system in front of the east trolley motor.     b. An approximate 40 foot fall on the trolley of crane 305 by utilizing an effective guardrail/entry system on the southwest corner.      c. An approximate 10 foot fall on the trolley of crane 305 by utilizing an effective guardrail/entry system on the southeast corner.    d. An approximate 25 foot fall for employees boarding crane 307 by utilizing an effective guardrail/entry system.       e. An approximate 40 foot on the trolley of crane 307 by utilizing an effective guardrail/entry system on the southeast corner.    f. An approximate 40 foot fall on the rescue walkway of crane 310 by utilizing an effective guardrail system on the west side walkway.    g. An approximate 10 foot 8 inch fall on the trolley of crane 315 by utilizing an effective guardrail system near the east side drum brake.    h. An approximate 40 foot on the trolley of crane 315 by utilizing an effective guardrail/entry system on the southwest corner.    i. An approximate 40 foot fall on the trolley of crane 315 by utilizing an effective guardrail/entry system on the northwest corner.    j. An approximate 40 foot fall on the trolley of crane 315 by utilizing an effective guardrail/entry system on the northeast corner.    k. An approximate 40 foot fall on the trolley of crane 315 by utilizing an effective guardrail/entry system on the southeast corner.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    During the week of November 2,  2015, and prior, the employer failed to ensure employees directed to perform preventative maintenance were protected from various fall heights, including but not limited to, the following cranes:      a. An approximate 40 foot fall while gaining access to crane 315. Employees walk from crane 310/318 across the crane runway and enter the crane without utilizing a personal fall arrest system.       b. An approximate 40 foot fall by ensuring employees do not have to climb over equipment on the east side which takes them above the guardrails of the trolley on crane 304.     c. An approximate 9 foot fall by ensuring employees do not have to climb over equipment on the southeast entry area which takes them above the guardrails  on of the trolley of crane 307.      d. An approximate 40 foot fall by ensuring employees do not have to climb over equipment on the north side rail which takes them above the guardrails on the trolley of crane 305.    e. An approximate 40 foot fall by ensuring employees do not have to climb over equipment on the south rail side which takes them above the guardrails on the trolley of crane 307.    f. An approximate 9 foot 6 inch fall by ensuring employees do not have walk in areas that take them above the guardrails on the southwest corner on the trolley of crane 314.    g. An approximate 40 foot fall by ensuring employees do not have to climb over equipment on the west side rail area on the trolley of crane 305.

During the week of November 2, 2015, the employer failed to ensure employees performing preventative monthly maintenance on crane 304 were protected from a fall of approximately 40 feet by utilizing a standard guardrail system on the south side walking platform.     b. During the week of November 2, 2015, the employer failed to ensure employees performing preventative monthly maintenance on crane 307 were protected from a fall of approximately 8 feet on the south side end truck by utilizing a standard guardrail system.    c.  During the week of November 2, 2015, the employer failed to ensure employees performing preventative monthly maintenance on crane 307 were protected from a fall of approximately on the south side auxiliary trolley end truck40 feet by utilizing a standard guardrail system.     d. During the week of November 2, 2015, the employer failed to ensure employees performing preventative monthly maintenance on crane 314 were protected from a fall of approximately 5 feet on the north side end truck by utilizing a fall protective system.     e. During the week of November 6, 2015, the employer failed to ensure employees performing preventative monthly maintenance on crane 314 were protected from a fall on the north side auxiliary end truck of approximately 40 feet by utilizing a standard guardrail system.    f. During the week of November 2, 2015, the employer failed to ensure employees performing preventative monthly maintenance on crane 315 were protected from a fall in front of the festoon on the trolley of approximately 10 feet by utilizing a standard guardrail system.     The TimkenSteel Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.23(c)(1), which was contained in OSHA inspection number 978613, citation number 01, item number 002 and was affirmed as a final order on August 21, 2015, with respect to a workplace located at 2401 Gambrinus Ave. SW, Canton, OH 44706.      The TimkenSteel Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.23(c)(1), which was contained in OSHA inspection number 984005, citation number 01, item number 001 and was affirmed as a final order on August 21, 2015, with respect to a workplace located at 4511 Faircrest St. SW, Canton, OH 44706.
