29 CFR 1926.1060(b): The employer did not provide retraining as necessary for each employee using ladders and stairways so that the employee maintained the understanding and knowledge acquired through prior training:  Residential house, on or about August 7, 2015, employees were to fall hazards of approximately 10 feet in height while using a ladder to access the first landing of the rooftop that was not at least 3 feet above the landing and demonstrated a  lack of understanding for ladder safety onsite.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Residential house, on or about August 7, 2015, employees were exposed to falls greater than 6 feet to the unpaved ground level below while removing and reinstalling asphalt shingles on the second level rooftop with a pitch of 8:12 without any fall protections measures in place.  The Quick Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 314279720, citation number 1, item number 1 and was affirmed as a final order on June 22, 2012, with respect to a workplace located at 1051 Thistle Meade, Burleson, TX. 76028.  The Quick Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 796001, citation number 1, item number 2 and was affirmed as a final order on June 11, 2013, with respect to a workplace located at 5509 Dimebox Drive, McKinney, TX. 75071.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Residential house, on or about August 7, 2015, employees were to fall hazards of approximately 10 feet in height while using a ladder to access the first landing of the rooftop and did not have the ladder at least 3 feet above the landing of the roof.  The Quick Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 314279720, citation number 1, item number 2 and was affirmed as a final order on June 22, 2012, with respect to a workplace located at 1051 Thistle Meade, Burleson, Texas 76028.
