Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) 406 Longbranch Circle, Liverpool, NY 13421, On or before 8/10/15: Employees were stripping and installing roofing materials on a 3 pitch roof at a measured height of 24 feet 7 inches without being provided with proper fall protection.
