29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    Location: site On or about 8/10/15, safe and proper access was not provided for 2 employees accessing the ladder jack scaffold.

Location: site On or about 8/10/15, employees were not protected from electrical shock and burns while working on a ladder jack scaffold where one of the aluminum support ladders was within 3 feet of an energized 240 volt power line.

High & Dry Roofing, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 332342, citation number 01, item number 001a, and was affirmed as a final order on 05/31/2012, with respect to a workplace located at 22 Lafayette Road, North Hampton, 03862.      High Top Roofing, (owned by Michael Cahoon) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA Inspection Number 306944265, citation number 01, item number 001, and was affirmed as a final order on 01/27/2004, with respect to a workplace located at the Corner of Putnam Street and Flaherty Lane, Manchester, NH, 03101.
