29 CFR 1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have the load engaging means fully lowered, controls neutralized, and the brakes set to prevent movement: a) On or about 07/20/15 - Dollar General Distribution Warehouse located at 4101 Lakeshore Parkway, Bessemer, AL, an employee operating a Raymond End Rider Pallet Jack, Model 8410, dismounted the vehicle and did not ensure the controls and brakes were set to prevent movement of the equipment, resulting in an injury.
