29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:    (a) Throughout facility; The employees were operating powered industrial vehicles without successfully completing a training course which included formal instruction, practical training and an evaluation of their performance.

29 CFR 1910.1200(b)(1): The employer did not develop and implement an employee training program to communicate information to their employees about the hazardous chemicals to which they are exposed, by means of a hazard communication program, labels and other forms of warning, safety data sheets, and information and training:    (a) Throughout facility; The employees were not provided with an adequate program communicating the hazardous chemicals to which the employees are exposed to such as but not limited to compressed gas cylinders containing oxygen, acetylene, propane and argon mixture; penetrating oil, lubricating oil, solvents and paints.
