29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle: (a) 3250 N Susquehanna Trail, York Pennsylvania - Two employees worked together to bend a piece of metal. The employer did not provide each employee with a foot pedal for activation of the Roundo AB roll bender to prevent accidental operation when an employee was in the danger zone, on or about August 4, 2015. (b) 3250 N Susquehanna Trail, York Pennsylvania - Employees operated a Roundo AB roll bender to bend a piece of metal. The employer did not ensure that employees were provided with adequate point of operation guarding, on or about August 4, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
